De alguma maneira os dados "duplicados" não são retornado.

Os relacionamentos N:M no Sequelize.js ignoram linhas duplicadas mesmo a query retorne os valores corretamente.

Uma possível solução é substituir o relacionamento belongsToMany para dois hasMany com a Tabela de Junção.

TestPlans.hasMany(models.TestPlanTests, {
        foreignKey : 'testPlanId',
        as: 'tests',
})

Tests.hasMany(models.TestPlanTests, {
        foreignKey : 'testId',
        as: 'tests',
})
